I should've known

Less than one hundred pages in (of an 800-page book), 2 major characters have died, a further 2 or 3 minor characters have died, and a character I was absolutely sure was dead resurfaces. This is Diana Gabaldon's Echo in the Bone. And yes, I should've known.
